My name is Nicole A. Smith, PT, DPT, GTS. I'm a Doctor of Physical Therapy who is passionate about helping you live your best life!  I have over 18 years of outpatient orthopedic experience and after years in traditional clinic settings I found myself frustrated as the demand to see more patients in shorter periods of time increased and insurance company control of patient care also increased. As a result, Gnarly Oak Physical Therapy and Wellness was created.  I work for you, not insurance companies, to provide exceptional, personalized, hands-on care to get you out of pain and  moving again so you can get back to the activities you enjoy most!

What Makes Us Unique.   A mobile/concierge physical therapy and wellness practice providing care in the comfort of your own home or office. No crowded waiting rooms or gyms! No sitting on hot packs while your physical therapist is treating multiple patients. No being passed off to techs to perform exercises you already know how to do.

What To Expect. The initial consultation is 60- 75minutes. After discussing your medical history, lifestyle and therapy goals a comprehensive evaluation will be performed not just on the area of pain or injury but a full body assessment. This holistic approach will include measuring your strength, range of motion, joint mobility, soft tissue mobility such as fascia and muscle, and posture. In addition, a Selective Functional Movement Assessment may also be performed (SFMA) to gain a clear clinical picture of your overall mobility and how compensatory movement patterns may be contributing to injury and pain. Each 60 minute follow-up treatment session will utilize manual therapy techniques, therapeutic exercises and patient education to address your injury or areas of concern.

We want to empower you with the knowledge, confidence and
guidance to help you heal yourself. An important part of your treatment plan will be teaching you self-treatment techniques you can do at home. In addition, we will regularly review, modify and progress your home exercise program. Lifestyle modifications may also be discussed to help you feel your best.

DO I NEED A PHYSICIAN REFERRAL?

Virginia is a Direct Access state allowing a patient to receive physical therapy services within a 60 day period starting the day of the Initial Evaluation before a script from a physician is required to continue treatment.

RATES

Gnarly Oak Physical Therapy and Wellness is a Fee for Service practice providing care in the Warrenton, Gainesville and Nokesville areas. The initial evaluation is $175 and 60 minute follow-up treatment sessions are $150.

 Many insurance policies have "out-of-network" benefits. A detailed invoice will be provided for you to send to your insurance company for possible reimbursement.  At this time we are only accepting Medicare patients for Wellness Services not covered by Medicare.
